Transcript request
To process a transcript request, we'll need the request in writing with the (former) student's signature.
You can download and print out our PDF Transcript Request form and mail or fax that to our office, or you can simply mail or fax us a letter with the following information:
- Name (if name has changed since graduation)
- Name at time of attendance
- Dates of attendance
- Social Security number or Student ID number
- Date of birth
- Address(es) where transcript(s) are to be sent
- Number of copies requested
